Hazelmere Nursing Home

Hazelmere provides nursing, care, and accommodation for up to 28 people, with 21 residents at the time of inspection. The service cares for mainly older people living with conditions including arthritis, diabetes, heart conditions, and dementia. The management team uses governance systems to review the service and achieve positive outcomes for residents. However, these systems have not been fully effective, as the home had not identified that some people lack specific care plans and risk assessments tailored to their individual health needs.
