Risk assessments contained insufficient detail and staff lacked clear guidance plans. Care plans related to medicines management were incomplete, leaving staff without essential information to support residents safely.

Additional issues included shortfalls in fire safety planning and drills, and concerns about staffing levels and support, limiting opportunities for meaningful interaction with residents. The physical environment required decoration and maintenance with no improvement plan in place. The registered manager and provider were not conducting regular audits to monitor care quality.
