Inspectors found that risks were not always consistently assessed and managed, with risk assessments and care plans often lacking information staff needed to provide safe and effective care. Medicine administration also presented concerns, as people were not always receiving medications safely or according to prescriber instructions. Governance and management systems were not effective in identifying and addressing these shortfalls in care delivery.
