Middleton Manor Care Centre, in Darlington, provides nursing and residential care for up to 83 adults, including people living with dementia, sensory impairments, mental health conditions or physical conditions. At the time of the assessment 34 people lived there. The CQC rates the home Requires Improvement overall, and it remains in special measures after a follow-up assessment found many previous breaches unresolved.

Inspectors found ongoing problems with medicines management, infection control, staffing consistency, risk management and activities, along with weak oversight and record-keeping. New breaches around dignity and respect, and failure to display the rating, were also identified. Feedback was mixed: some families praised kind, improved communication, while others raised concerns about mealtime support, cleanliness and limited stimulation.
