While systems were in place to assess and monitor quality and safety, several improvements were needed. Staff documentation required updating to ensure consistent, safe care delivery. Incident forms were completed but lacked analysis, follow-up, or overview to prevent recurrence or mitigate risk.

Incidents and accidents were not consistently reflected in residents' care plans, and staff were not analysing trends in incident patterns or monitoring the effectiveness of pain relief medication or requests for as-needed medicines.
