While most recruitment processes were safe with appropriate pre-employment checks completed, some recruitment records lacked complete employment history details. Care plans and risk assessments were inconsistent across the service, with some people's risks properly assessed while others were not, indicating gaps in the consistency of care planning and risk management practices.
