However, inspectors identified significant gaps in care monitoring and planning. Residents with skin concerns were not receiving documented daily skin checks as stated in their care plans, leaving potential deterioration unobserved. Those experiencing constipation lacked formal management plans to prevent complications. Not all risks had been properly identified or assessed, potentially exposing residents to harm. Additionally, some care plan guidelines contained outdated information, which could affect the quality of support provided to residents.
