Swan House

Swan House is a care home providing personal care for up to 45 people, with both residential and nursing care available. The home is split across two floors, with the lower floor housing residential care and the upper floor providing nursing care. Some residents are living with dementia and have access to their own bedrooms along with communal lounges. At the time of inspection, 40 people were living in the home.

The service was not safe. Risks to people were not adequately monitored or reviewed following incidents or changes. Care plans and risk assessments lacked necessary detail, and safety actions were not consistently followed. People did not always receive pain relief when needed. Staff did not ensure people were protected from harm.
