Care planning was inconsistent, and governance systems had failed to identify these problems, continuing a breach from the previous assessment.

Despite these shortfalls, people and relatives spoke positively about staff, describing them as attentive and caring, and felt safe living there. Infection control was effective and the environment was clean. Leaders responded promptly to concerns raised, though stronger oversight is needed to prevent issues recurring.
