The service was rated 'Requires Improvement' following the previous inspection in August 2017, when breaches were found including failure to notify CQC of all required incidents. Risk assessments were incomplete and not all practicable steps had been taken to manage identified risks to people. Records relating to care, risk management and complaints were not maintained sufficiently or accurately, and the provider's quality monitoring systems had failed to identify and address these shortfalls.
