During the inspection, governance systems were found to be inadequate at identifying issues and driving improvements to service quality and safety. Staff demonstrated a lack of understanding regarding requirements for actions and records needed to keep people safe.

Inspectors identified concerns about medicine management, finding that people's prescribed medicines were not always handled safely. Issues with the administration of thickening agents for fluids presented additional safety risks that governance systems had failed to address.
