Brookfield Support Centre

Brookfield Support Centre is a residential care home providing personal care for up to 39 people, with a focus on intermediate care services. The service supports people who are ready to be discharged from hospital but need further recovery time, or those managing difficulties at home due to illness or reduced mobility. Accommodation is provided on the ground floor with full access to adapted facilities.

Staff support people to have maximum choice and control of their lives, working in the least restrictive way possible and in their best interests. The service's policies and systems reflect this commitment to person-centred care.
