Hannahwood Transitions

Hannahwood Transitions provides personal care and support for up to 23 adults with learning difficulties and autism, with 15 people living at the service at the time of inspection. The service operates a model of care that maximises people's choice, control and independence. Staff are committed to supporting people according to their preferences and help them receive medicines safely and as prescribed. People are enabled to access health and social care support in the community, and risks associated with care are assessed and included in support plans to guide staff on appropriate actions.

The registered manager and staff demonstrate a clear commitment to person-centred care and have good knowledge of the service and the needs of the people they support. Staff support people in the least restrictive way possible, in line with their best interests, and the service's policies and systems support this practice.
