Risks were not always properly recognised or mitigated, and there were errors in weight management calculations that went undetected. Best interest decisions regarding the use of bedrails were not consistently documented, though the deputy manager took immediate action to address this. The home lacked effective systems to properly monitor the safety and quality of care being provided.
