The service provides care through two pathways, including mental health rehabilitation. Inspectors found that environments were not always safe or well maintained, and staff did not consistently assess and manage risks to people using the service. The use of restrictive practices was not always managed safely, and some staff lacked required training. Care plans for people in the mental health rehabilitation pathway were not consistently recovery-oriented or holistic, with records not clearly showing how the service assessed or met people's full range of needs. Some aspects of care delivery did not align with national guidance and best practice standards.
