The service provides care through two pathways, including mental health rehabilitation. Care plans for people in the mental health rehabilitation pathway were not consistently recovery-oriented or holistic, with records not clearly showing how the service assessed or met people's full range of needs. Some aspects of care delivery did not align with national guidance and best practice standards.
