Inspectors found the service was not well-led, with a lack of effective governance systems to ensure regulations were being met. The provider had not maintained adequate oversight to keep people safe. There were insufficient systems in place to ensure the environment and equipment were safe and clean, and poor infection control practices at the service placed people at risk of harm.
