Inspectors found significant gaps in how risks to residents' health, safety and welfare were assessed and managed. Safety concerns extended across several areas, including individual care plans, environmental conditions, and infection prevention and control measures. The home's quality assurance systems were not consistently effective at identifying and mitigating these risks, with insufficient checks being carried out across the environment during periods when management was absent.
