Oakmeadow community support centre is a residential care home providing personal care for up to 32 people, divided into two sections: Oakmeadow on the lower floor offering intermediate care, and Hawthornes on the upper floor providing rehabilitation support. The home aims to prepare people to return home following hospital discharge or to prevent hospital admission. The service also supports people with personal care in their own homes after discharge from hospital or the residential facility.

Inspectors identified breaches in staff training, supervision and appraisal across both the residential and community services. Staff reported they had not always received appropriate training to fulfil their roles effectively, and training was inconsistent throughout the organisation. While most people's care and support needs were reflected in their plans, not all support plans were complete and accurate.

Details about end-of-life care plans for community-based service users and health-led therapy information for residential residents were not always available, which represented a further regulatory breach.
