Oakmeadow Community Support Centre

Oakmeadow community support centre is a residential care home providing personal care for up to 32 people, divided into two sections: Oakmeadow on the lower floor offering intermediate care, and Hawthornes on the upper floor providing rehabilitation support. The home aims to prepare people to return home following hospital discharge or to prevent hospital admission.
