The Elms

The Elms accommodates up to 37 younger and older adults with physical disabilities or dementia in one adapted premises with shared facilities. At inspection, 30 people were using the service. Safety and wellbeing were compromised by shortfalls in infection prevention and control, unsafe medicine management, and incomplete risk assessments. While staff appropriately managed accidents, incidents and safeguarding concerns, no monitoring system existed to support learning and prevent recurrence. Care plans lacked timely completion and detailed guidance for meeting people's needs, and healthcare professional advice was not clearly recorded, leaving staff without up-to-date information. Staff did not consistently support people to have maximum choice and control of their lives in the least restrictive way possible. Quality assurance systems were ineffective in maintaining safety and quality standards. The provider was working with relevant professionals to improve the service.
