Avocet Trust 22a-26 Middlesex Road provides personal care for up to six people with learning disabilities and/or autism across four adapted bungalows. At the time of inspection, all six available spaces were occupied.

Inspectors identified significant safety concerns, including poor oversight of fire safety issues that put residents at risk. The provider's quality assurance systems had not effectively monitored the safety and quality of the service, meaning these hazards were not being adequately managed or addressed.
