Regular checks now ensure staff follow training and health and safety guidance, with systems in place to monitor pressure mattress settings, investigate unexplained injuries, manage oxygen safety, and address choking risks. However, the provider's quality monitoring systems were not being consistently and effectively used throughout the home.
