Inspectors found that care records were not always completed or accurate. Some care plans, risk assessments and best interest decisions lacked the specific details required about people's care. The home's audits did not identify the recording issues that inspectors found during their visit.

The provider should review their governance processes to ensure record keeping is monitored effectively.
