Trentham Care Centre provides nursing and personal care to 96 people across five units, serving younger and older adults with dementia, mental health needs, and physical disabilities. At inspection, the home was operating four of its five units.

The inspection found significant concerns with medicines management. Staff did not consistently follow best practice when administering medicines, and not all medicine errors were reported according to the provider's procedures, meaning people did not receive their medicines safely. Quality monitoring processes were inadequate to identify or address these issues and other inconsistencies in care.

Staff interactions with residents were inconsistent. While some people described being supported by caring and kind staff, others did not receive empowering and valuing interactions. The provider had assessed risks associated with people's care and support, but lacked effective oversight to ensure these assessments translated into safe, consistent care experiences for all residents.
