Environmental risks were not always properly identified or monitored, and the home did not consistently review accidents and incidents to prevent future harm. Quality assurance systems were ineffective, with audits not regularly completed or kept up to date.

Despite these issues, people felt safe at the home and reported positive experiences. Relatives felt their loved ones received good care, and staff numbers were adequate to meet residents' needs. The home had systems in place to gather feedback and address complaints from residents and families.
