Inspectors identified serious safety concerns. While staff demonstrated good knowledge of residents' needs and provided safe support in practice, critical information was not properly documented in care records. Risk assessments lacked specific details about how people should be supported safely. The physical environment also posed hazards—windows were not appropriately restricted and fire safety barriers were absent, creating risks of falls from height and other injuries that residents were not adequately protected against.
