The home failed to conduct appropriate checks before deploying agency staff, putting people at risk. Choking risks were not managed safely, and thickener was introduced without proper Speech & Language Therapy Team advice, creating potential harm.

Fire drills had not been performed according to the provider's own policy. These significant issues had not been identified by the provider or registered manager, indicating that quality assurance processes were ineffective and failing to safeguard residents.
