Inspectors found quality had declined since the previous assessment, with continuing breaches around person-centred care, safe care and governance, plus new failings in safeguarding, staffing and notifying the CQC of incidents. Risks, including repositioning and medicines, were not managed safely, and leaders lacked effective oversight. Feedback from people and relatives was mixed, with some praising kind staff while others described unanswered call buzzers, poor cleanliness and neglect.
