However, the service has significant quality concerns. Risk assessments of the physical environment were not being conducted properly, leaving people vulnerable to infection due to poor bathroom maintenance and unsafe practices like sharing communal soap. Infection control audits were not completed, and general audits failed to identify issues such as a carpet with strong urine odors. Recruitment processes were inadequate, with staff hired without properly documented checks including full work histories, references or interview records. Staff had not received recent training in fire safety, safeguarding or medicines administration, though management reported informal in-house training had occurred.
