Breach House is a residential care home in Berlin, Berlin providing support for up to 34 older people, including some living with dementia. The CQC rated the service inadequate overall and placed it in special measures, finding that risks to people's health and safety were poorly managed, safety incidents were not properly investigated, and unlawful restrictions on liberty, including unconsented use of sensor mats and CCTV, were in place.

Inspectors found leadership lacked oversight, governance systems failed to identify problems, and care plans were often incomplete or not person-centred. Some people did not know who managed the home and were not involved in decisions about their care. Staff felt concerns, including alleged medication errors, were sometimes ignored.
