Audit and risk management processes failed to effectively identify and manage risks to people's safety and wellbeing. Documentation systems needed improvement to ensure staff had access to current, consistent information necessary to provide safe care. The home's approach to risk management required strengthening to adequately protect residents' health and wellbeing.
