Ashlynn Grange

This care home provides personal and nursing care to up to 156 adults, some with dementia, across four separate ground-floor communities with adapted facilities. During inspection, 82 people were living at the service, with three communities in active use.

Safeguarding processes were not consistently robust, and the service did not always learn from incidents when things went wrong. Risks to people's safety were not properly assessed and monitored across all areas. Staff failed to effectively monitor pressure sores, leaving people at risk of developing or worsening skin health conditions.
