Alt Park Nursing Home

Alt Park Nursing Home provides personal and nursing care for up to 35 people aged 65 and over, with 33 residents at the time of inspection. While the provider had made some improvements to governance, audits of care plans identified concerns that were not followed up to verify completion of actions. Medication audits failed to identify issues discovered during the inspection itself, leaving gaps in oversight. Some care records were incomplete, inaccurate, or lacked sufficient detail to guide staff in supporting people appropriately, and records relating to complaints and incidents required further development.

Risk assessments contained conflicting and confusing information that could put people at risk of avoidable harm. Following feedback after day one of the inspection, the provider implemented mattress checks and took action on other safety concerns. However, inconsistencies in governance and record-keeping meant people remained at risk of receiving care that did not fully meet their needs.
