The environment was not consistently clean and safe, and care documentation lacked current information about people's needs and risks to guide staff properly.

Staffing levels were inadequate to meet residents' needs and monitor their safety effectively. Medicines were not managed safely in line with national guidance. The home also needed to strengthen staff recruitment processes to ensure appropriate background checks were completed.
