The Evergreens Lodge

Riverside Nursing Home provides personal and nursing care for up to 39 people aged 65 and over, including those living with dementia. At the time of inspection, 29 people were living at the service.

The provider and staff team had worked hard to improve the service, with many improvements underway. However, the home did not consistently assess, mitigate and monitor the risks people faced to their health and wellbeing. While people's care needs were assessed and care was planned, staff did not always follow the care plans that had been established.
