Managerial oversight required improvement. The provider's systems and processes for monitoring quality and safety remained ineffective and had not identified the shortfalls found during inspection. Policies and procedures were not always followed, and the management team needed to increase their knowledge of regulatory requirements to ensure compliance. Risk management also required improvement, with staff lacking information needed to provide safe care.
