Positive Step

Positive Step is an adult social care assessment unit accommodating up to 35 people. The service receives referrals from Doncaster Royal Infirmary's integrated discharge team and provides short-term accommodation for people requiring comprehensive assessment. Staff work to build residents' confidence and skills to enable them to return home or move to more suitable placements. The service contributes to the hospital discharge pathway, helping to reduce hospital stays and prevent delayed discharge from acute beds. Its core purpose is to reduce or delay admissions to long-term care and prevent hospital readmissions by facilitating safe discharge with appropriate community and home support tailored to individual needs.
