Oak Tree Mews, in Gloucestershire, is a care home without nursing for up to 20 people, including some living with dementia; 14 people lived there at the time of the assessment. The CQC rates it Requires Improvement overall, with the safe key question now rated Inadequate. Inspectors found ongoing failures in medicines management, infection control, staffing and risk management, alongside a poor staff culture and language that could be emotionally damaging.

Some improvements were noted around mental capacity assessments and pressure ulcer care, but the provider remained in breach of four regulations and two new breaches were identified, relating to staffing and dignity and respect. Some staff were seen to be caring, though inspectors also observed poor practice around meals, activities and comfort, and relatives gave mixed views on progress.
