The service received an inadequate rating following concerns about compliance with mental health regulations and medication management practices. Staff failed to follow the Mental Health Act Code of Practice when caring for a patient in long-term segregation, with the patient unable to freely mix with other patients on the ward for five months. Despite this extended period of isolation, staff had not recognised the situation as segregation.

Significant gaps were identified in care planning for patients receiving medicines covertly, with no documented plans detailing how staff complete this safely. Patients receiving medicines through percutaneous endoscopic gastrostomy (PEG) tubes also lacked detailed care plans to ensure medications are administered according to best practice guidance. Managers failed to ensure these essential safeguards were in place.
