The provider's governance and oversight arrangements show progress, but improvements are not yet consistently embedded throughout the service. Care plans do not always contain relevant information about people's current care needs or changes to these needs. Proactive safety measures that staff should take are not consistently recorded in care plans, leaving gaps in how staff understand and respond to individual risks.
