Care is planned around the person's desired outcomes, with tools introduced to help them make choices about their care. The home provides a timetable of meaningful activities and ensures healthcare needs are met through regular check-ups and robust monitoring systems. The person benefits from a dedicated keyworker who knows them well, alongside regular involvement from their advocate and relatives.
