Risk assessments did not adequately address how to manage violence and aggression, dangerous equipment was accessible to residents, and bathroom door locks could not be opened by staff in emergencies. Infection prevention and control measures were insufficient, and quality assurance systems were not consistently effective. Pre-admission assessments were also incomplete in covering residents' needs.
