Signature at Chorleywood accommodates up to 100 residents requiring personal care, including adults living with dementia. The facility is purpose-built across three floors, with a dedicated "reminiscent" community section providing additional support for people with dementia. At inspection, 68 people were using the service.

Significant concerns were identified regarding oxygen therapy management. The home lacked adequate processes and safety checks for storing and managing additional oxygen, and staff had not received training on oxygen therapy or its associated risks. Management and the provider had not ensured proper oversight of these critical safety procedures.
