This residential care home accommodates up to four children and young people requiring personal or nursing care alongside therapeutic psychological support. At the time of inspection, four young people were using the service.

Inspectors identified significant concerns around consistency in managing self-harm and risk. The 'Managing Self-Harm Policy' had not been reviewed in line with best practice guidance, and there was no consistent therapeutic approach when young people attempted self-injury. Young people's risks had not always been clearly assessed and monitored, and safeguarding concerns and incidents were managed inconsistently. While some safeguarding matters had been reported and investigated appropriately, this process had not been consistently applied across the service. There was also uncertainty about whether staff had received appropriate safeguarding training levels.

Additional gaps were found in mental capacity assessments, which had not always been undertaken when needed. Consent and capacity were not consistently sought, reviewed or documented across the home.
