17 York Avenue provides accommodation and personal care to up to four people with learning disabilities, mental health needs, and physical disabilities. At inspection, all four available places were occupied. Staff assess residents' risks and needs to develop care plans addressing their support requirements. However, inspectors identified gaps and inconsistencies in the paper-based records, including missing dates, outdated information, and inappropriate templates. Self-medication care plans lacked sufficient detail about the level of staff support needed, and risk assessments were not consistently reviewed when residents' medicines changed. The manager has begun taking steps to address these record-keeping issues.
