Hazelmead Residential Care Home

This residential care home provides personal care for five people with physical and learning disabilities in a bungalow designed to support those with physical disabilities. Staff knew people well and provided person-centred care with clear guidance documented on how to care for each person. Staff supported people to have maximum choice and control of their lives in the least restrictive way possible, and enabled access to specialist health and social care support in the community. Medicines were managed safely, with clear guidance on when to administer 'when required' medicines for people who may not be able to request them themselves. However, the practices around documenting decisions were not in line with guidance.
