The provider's quality assurance systems did not fully identify all concerns, and some records lacked accuracy and detail. However, when issues were brought to the provider's attention, they responded promptly and effectively to address them. Staff demonstrated understanding of how to assess and manage risks to people's health, safety and well-being.
