Support plans and risk assessments were not regularly reviewed or updated, and staff sometimes lacked information needed to support people safely. Additionally, infection prevention and control procedures were not being followed in line with the provider's policies. The attitudes and behaviours of leaders and staff did not consistently ensure people were leading confident, inclusive, and empowered lives.
