St Leonards Care Home provides accommodation and personal care to up to 45 older people, including those living with dementia, across two specialist units. One unit focuses specifically on dementia care. Each unit has communal sitting and dining areas with access to enclosed outdoor space.

Inspectors found that risks to people were not consistently identified, mitigated, and reviewed. Staff recruitment practices were inadequate and staff competencies were not properly assessed for their roles, including medicine administration, though medicines were ultimately given as prescribed. The service did not consistently support people to have maximum choice and control over their lives, nor did it apply least restrictive practices. While auditing identified shortfalls in service provision, timely action was not taken to address these findings and mitigate risks to people.
