Safety concerns were identified, including incomplete staff training and inaccurate care records with conflicting risk documentation. The environment had cleanliness and maintenance issues. Residents were not always supported to have maximum choice and control of their lives, with staff not consistently enabling their involvement in decision-making about their care and support.
