Inspectors identified some gaps in monitoring records. Staff had not consistently recorded night checks as specified in people's care plans, and there were instances where assisted repositioning for people at risk of skin pressure damage had not been documented as required. The provider confirmed that care monitoring systems would be further strengthened to identify and address any gaps in people's care and support quickly.
